Application forms and instructions

To apply to CMUDI, you will need to use the standardized forms (2005) and additional CMUDI forms with instructions. If you utilize the Microsoft Word downloads, you will be able to type the information directly onto the forms by opening the document in Microsoft Word or another text edit software. To be sure that your word processing program and printer provide the correct format, please compare it with the Adobe Acrobat format (PDF version). If necessary, you can download the free Adobe Acrobat Reader from the following website: www.adobe.com/products/acrobat/readstep2.html.

Program Completion

To complete the program the intern must successfully complete the
prescribed program of learning experiences and meet or exceed
established standards, as defined by the core competency statements.

A. The prescribed program of learning experiences has been designed to
meet the Standards of Education of the American Dietetic Association and
will be completed in its entirety.

B. The intern will be evaluated at specified intervals as delineated by
Policy and Procedure 500.1, Evaluation of Interns. An acceptable
standard of performance must be achieved.

C. Successful completion of the Central Michigan University Dietetic
Internship will culminate in receipt by the intern of The American
Dietetic Association Verification Statement (6 originals) completed by
the program director.

What is computer matching?

Applicants to the CMUDI must participate in computer matching. Applicants should visit the D&D Digital Systems website (see address below). This request should be made to allow turnaround time for submitting by the postmark deadline. There is a $50 charge for computer matching (payable to D&D Digital Systems) that is due with the applicant's prioritized ranking. The website is: http://www.dnddigital.com

Application deadline and other important dates

September 25, 2008 is the postmark deadline for applications to the January 2009 internship class. It is also the deadline for the computer matching fee to D&D Digital Systems. November 17, 2008 is the appointment day for the January 2009 program. On this date, applicants who are matched to the CMUDI must e-mail, fax, or telephone the CMUDI Program Director, Ann F. Jay, M.S., R.D., F.A.D.A. to confirm their acceptance or rejection of their match.

Financial assistance
Federal student loans and other financial aid and scholarships are available to interns enrolled in the CMU College of Graduate Studies as regular, degree-seeking students.
